All meeting attendees must sign our waiver of liability before your meeting. All attendees who complete this waiver online will receive an email confirmation of waiver completion.

Download, print, and complete this sign-in form as your meeting attendees arrive to your meeting. This will be returned to LBUMC when you complete the Post-Meeting Checklist.
